Here you are posing a scenario a bit too literally, about the workings of karma, that it would enforce a tit-for-tat replay quite closely of a previous karmic dilemma. While that can in fact happen, it is more likely there will be some resemblance to prior circumstances but with wide variations in time, place, circumstances, and age at which a karmic comeuppance will manifest.
So the idea of a karmic consequence visited on a young child still wholly unaware of even right and wrong and having such an immature mind, could not possibly be held to account in the same way as an adult. So a karmic consequence might seem in fact wasted in causing the death of a three-year-old who has not yet really understood who they are, what life is all about, and had an opportunity for enough independence and wherewithal to even make life choices for itself, and thus have any chance to forestall a karmic penalty should that come along. What is served by someone dying so very young is that, in losing the balance of their life and its learning opportunities and growth potential, it is a major penalty. In effect, the person is being denied access to the playground and will have to return to the light to regroup and start over, in planning another incarnation as the only way to truly reckon with the karmic burden, as it cannot be repaired within the light but must happen in the physical where it was experienced originally. So this is an object lesson in what can happen to get someone in trouble, and stay behind the curve even in life after life, and can serve as a warning to take your life seriously, to safeguard yourself through making wise choices, and learning all you can to do so effectively.
Once a karmic potential like this is launched, it does indeed take at times a miracle, through a third-party rescue, to save them. That as well is why you are here as a divine human, to help your brethren overcome their karmic backlog, and yours as well, whatever form it might be exhibiting at the moment, as well as all the potential that is looming for things to arise again like physical illnesses, life disappointments and failures, accidents and injuries, troubled relationships leading to chronic unhappiness and early death from accumulated stress. The takeaway message here is even with a severe karmic liability there is always hope, because you have the power within to partner with the divine and head off such disasters, if you get going and use your time wisely and well by attending to your healing needs before the problems arise, preferably, because when they do it might well be too late.
